Step 6: Parent Release Form for Media Recording

I, , the undersigned, do hereby grant or deny permission to 6th Sense-Potter's Wheel Tennis Academy to use the image of my child, , as marked by my selection below. Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but ma y not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the 6th Sense-Potter's Wheel Tennis Academy Website.

Deny permission to use my child's image at all.

Unrestricted usage: I give unrestricted permission for my child's image to be used in print, video, and digital media. I agree that these images may be used by 6th Sense-Potter's Wheel Tennis Acdemy for a variety of purposes and that these images may be used without further n otifying me. I do understand that the child's last name will not be used in conjunction with any video or digital images.

The total fee will be required at the time of your reservation to guarantee your stay.
If your child has a chronic medical condition such as diabetes, severe allergies or others illness, please contact 6th Sense - Potter's Wheel Tennis
Academy before you make your travel arrangements.
You acknowledge and agree to assume and be fully responsible for any and all property damage to the room or any other facilities used at 6th Sense -
Potter's Wheel Tennis Academy .
6th Sense - Potter's Wheel Tennis Academy is not responsible for any injuries, lost or stolen articles or money during your sta y. Please DO NOT bring
valuable items.

The credit card number on file will be charged for an unpaid balances, damages, extension fees and/or expenses incurred during the stay. Prices are
subject to change without notice. For international credit card, we will charge 5% of the total amount.
